OMF-TPL
Form Template: OMF-TPL
This template is used to precisely position the anchorage assembly on the form prior to the concrete pour. It helps ensure that the assembly is positioned correctly with relation to the forms. While a template is included with each OMFSL or OMFAB assembly, these are sold separately for replacement or convenience.
Universal Template Locator: OMF-UTLA
This optional layout tool is designed to help the installer accurately space two OMFSL or OMFAB anchorage assemblies on the forms prior to the concrete pour. Anchor bolt location is critical to a trouble-free moment frame installation. By using the UTLA to position both assemblies at once, the installer helps ensure that the anchor bolts will be spaced correctly to accept both moment frame columns.

- Spacing is locked into place when the pins are set, eliminating variability due to handling or transportation

How to use:
- Assemble the inner tube and slide on the two outer sleeves with all markings facing the same way.
- Determine where the first anchorage assembly will be positioned and fasten it to the forms using duplex nails or screws.
- Adjust the sliding ends of the Universal Template Locator (UTLA) to the correct column and beam combination, as marked on the center tube, and lock them in place with the locator pins. Column and beam model numbers are on each component.
- Place one of the end plates of the UTLA over the top of the anchor bolts on the form-mounted assembly.
- Lay the UTLA across the forms so that the free end plate is positioned where the second column will be located.
- Holding the end plate of the UTLA up to the top of the form, locate the center line on the plate and mark it on the outside of the forms. Remove the UTLA from the forms.
- Position the second anchorage assembly so that the center line on the assembly template aligns with the line marked on the forms. Fasten the anchorage assembly template to the forms.
Extension Kit
Extension Kit
The Strong Frame anchorage extension kit extends the anchor
rods in the OMFSL and OMFAB anchorage assemblies to allow for
anchorage in tall stemwall applications where embedment into the
footings is required. Made from ASTM F1554 Grade 36 rod or ASTM
A449 rod, the extension kits feature heavy hex nuts that are fixed
at the correct position to go underneath the shear lug or template
and a “No Equal” (≠) head stamp for identification. Coupler nuts
are included with each kit. Kits available hot dipped galvanized
for corrosion protection when required, lead times apply.
Installation – OMFSL
- Remove original rods from the anchorage assembly.
- Insert extension rods (as shown) and fasten with 5/8" nuts provided.
- Cut bottom of rod to desired length so that the shear lug is flush with top of concrete.
- Install original anchor rods onto the bottom of the extension rods using the coupler nuts (provided). Tighten rods so that both ends are visible in the "witness hole" openings.

Installation – OMFAB
- Remove original rods from the anchorage assembly.
